Teens Against Tobacco Use - (TATU)
For use in Wisconsin by American Lung Association of Wisconsin certified
T.A.T.U. facilitators only.
Please complete the
T.A.T.U. Material Order Form and return to us at least
two weeks prior to your teen workshop.
UPCOMING DATES AND LOCATIONS
After the upcoming training, please submit the Teen Workshop Confirmation Form and indicate how many teens you've trained and their anticipated activities. Once we receive this form, you will be sent Certificates of Completion for your teens.
Upon completion of the T.A.T.U. teen workshop, please fill out the top portion of the Youth Presentation and Community Action Report Form and pass it along to the teen advisor/contact person to complete the remaining portion. This completed form will indicate what activities were completed, whom the teens presented to, which grade levels, and how many students they were able to reach. Please use one form for each school or organization the T.A.T.U. teens reach.
